If you have any of the affected SureStep®Pro® Test Strips, you may now return them to LifeScan for a credit and place a new order for replacement product. Note: This is not a mandatory market removal of affected product. Facilities may choose to use up their remaining supply of affected SureStep®Pro® Test Strips as long as their “Critical High” protocol threshold is no more than 400 mg/dL. In addition, all staff should be instructed to confirm via an alternate means or a lab test any result in excess of 400 mg/dL. If the ability to confirm results is not readily available, you can guide preliminary treatment by conducting a clinical assessment.

  1. If you wish to make a return, please confirm that you have affected SureStep®Pro® Test Strips by clicking here. Only SureStep®Pro® Test Strips with affected lot numbers are eligible for return.
  2. If you have affected product you wish to return, call 336-499-8180 for a Returned Goods Authorization (RGA) number and product return instructions. 
  3. When we receive your returned product, we will issue a credit memo for an amount equivalent to your invoiced price of the remaining returned product. No deductions will be allowed. 
  4. Place a new order for replacement test strips. All new shipments of SureStep®Pro® Test Strips will have expiration dates of November 2011 or later. With these strips, you will once again be able to set your “Critical High” threshold, per your normal procedures, up to 500 mg/dL.
