A Letter To LifeScan Customers:
--PRINTING ERRORS IN SOME OneTouch® ULTRA METER OWNER'S BOOKLETS--
If you use a OneTouch® Ultra Blood Glucose Monitoring System to test your blood glucose, recommend it to your patients, or distribute and sell it to customers, please read the following important information.

Some OneTouch Ultra Owner's Booklets Contain Printing Errors
LifeScan has recently determined that a small number of OneTouch Ultra Owner's Booklets contain printing errors. Of the approximately 5,300 Owner's Booklets that could potentially be affected, it is estimated that 16 Owner's Booklets actually contain printing errors. These booklets were included in several lots of OneTouch Ultra System Kits that were shipped from LifeScan to retailers in late October 2001 through early December 2001.

What To Do If You Have An Affected Owner's Booklet
While the incidence of these printing errors is extremely low, we recognize that some of the affected OneTouch Ultra Owner's Booklets are in the hands of customers. If you have an owner's booklet with missing information, please call our 24-hour LifeScan Customer Care line at 800 227-8862 for a free replacement. You can also access the complete OneTouch Ultra Owner's booklet online by visiting: www.lifescan.com/booklets/.

The "missing" information in the affected Owner's Booklets is included in other consumer instructional materials such as the OneTouch Ultra Test Strip Insert and the OneTouch Ultra Quick Start Guide.
